In this post, we are sharing with you the Official Evaluation Form for the Certification of Participation on  the Virtual Launch of the 'Disability-Inclusive Teaching in Emergencies (DTeEm) Online Course' Phase II in the Philippines last 25 April 2022.

Kindly take note of the following:

1. Please make sure that all answers in Evaluation Form below are correct and accurate. The DepEd-International Cooperation Office is not responsible for the typographical errors submitted in this form.
2. Only those who submitted the form will receive a certificate.
3. The Certificate of Participation will be sent to the provided email address two (2) weeks after the event. All follow-up/s and/or inquiries regarding the issuance of the Certificates will not be accommodated.
