Official Evaluation Form Link for DepEd Teaches Episodes 19-22 (4-Day Free Webinar)

In this post, we are sharing with you the official Evaluation Form for the 4-Day Free Webinar – Episodes 19-22 (May 6, 20, 27 and June 3, 2021) by DepEd Teaches. Please see the link at the bottom of this page.

The Evaluation Form (link) for Episodes 19-22 will be open until June 5 (Saturday), 11:59 PM.

Participants are required to successfully accomplish the whole evaluation process to be eligible to receive Certificates of Appearance and Participation. This includes obtaining a 75% score in the content evaluation part of this form.

By completing this evaluation process, the participant agrees to the conditions set by the organizers in issuing the said certificates.

Reminder: You only have one chance to take the Evaluation Form. So, be sure that you can answer all the questions given in the assessment or evaluation form . If you are now sure then you can still watch the replay videos of the 4 Episodes 19-22 via DepEd Teaches official FB Page (https://web.facebook.com/watch/TeachesDepEd/)
