DepEd: Adjusted School Forms are now available for downloading in LIS

Just In! Latest Update in DepEd's Learning Information System posted last May 10, 2021.

This is good news to all teachers. There's no need to create or alter the existing forms because the adjusted School Forms are already added and can be downloaded now in the LIS.

Important Reminders:

• In Compliance with the DepEd Memorandum No. 14 s. 2021, the adjusted school forms are now available for downloading through this link:https://bit.ly/33G4MtT

•  New added label for SF1

        - Age as of October 31, 2020

        - Learning Modality

• New added label for SF2

        -Learner Attendance Conversion Tool

        -LACT1

        -No Longer Participating in Learning Activity (NLPA)

• New added label for SF3

        -Module/Reference/Code

• New added label for SF4

        -No longer Participating in Learning School (NLPA)

So, visit or log-in to your account now and download all the forms: New SF1, 
New SF2, New SF3 and New SF4.
